About Yellow Notetaker Forms
You, the SSD student, must complete the first 2 lines on the yellow form that says "To The Student."
Ask the professor to pass the notetaker form around in class. Professor instructions are included at the top of the yellow form. Make arrangements with your professor to pick up the yellow form. You should return it to the Services for Students with Disabilities (SSD) Office at 250 South Main Street, Suite 300, as soon as possible so that we can contact the volunteer notetaker.
The Notetaker Coordinator in the SSD Office will contact you by e-mail and let you know when the notetaker will start and what day during the week he/she will be copying notes. You should pick up your notes the SSD Office on a weekly basis.
Even if no one volunteers to share their notes, you still need to return the yellow form to the SSD Office (250 South Main Street, Suite 300) so other notetaking arrangements can be made.
If the professor tells you that a notetaker has already been found for another SSD student in the same class, you still need to return your yellow form to the SSD Office and let us know about this. We will need to locate the notetaker for that class and let them know they need to make additional copies of their notes.
We do not know who gets notes unless the yellow notetaker form is returned!
