Procedures for Alternate Text Formats


Students requesting textbooks in alternate format (i.e. tapes, CD) will be required to complete a Request for Alternate Text Format form: (Word 97-2003 Doc) or (Adobe PDF) for each text you wish to receive in an alternate format. The request forms should be turned in to the SSD Office at 250 South Main Street, Suite 300, along with a copy of the class syllabus. NO REQUESTS FOR ALTERNATE TEXT FORMAT WILL BE COMPLETED UNTIL A REQUEST FORM IS RECEIVED.

1. You, the student, must present disability documentation to Services for Students with Disabilities (SSD).

2. Documentation must verify the need for alternate text formats as an appropriate accommodation.

3. You are responsible for providing textbook information to SSD. This information should include: name of text(s), author(s), publisher, edition and ISBN number. A copy of the class syllabus is also required. Please use the Request Form (Word 97-2003 Doc), or (Adobe PDF) for providing textbook information.

4. Once SSD has all the necessary information, we will process your request and you will be provided with the requested alternate text.

5. SSD will loan you a tape recorder, software, etc. on a per semester basis as needed.

6. You are responsible for picking up your alternate text(s) at the SSD Office unless alternative arrangements have been made.

7. If you fail to pick up your alternative text(s), this accommodation may be denied, depending on the situation.

8. Poor quality alternative text(s) or missing alternative text(s) should be reported immediately to SSD (Kelly Woodward - kwdward@vt.edu ).

9. You MUST return tapes, CDs, software, recorders, and any other material you have borrowed from SSD offices at the end of each semester.

10. You will be notified by email twice to return your materials. If these materials are not returned, your third notice will be an invoice for the missing materials. You will not be allowed to receive equipment from our offices the following semester until this invoice is settled, and a Hold will be placed on your account.
